- An initial assessment summary after the first visit
- Progress reports at plan review, and a discharge summary

Funding we work with
- NDIS
- Self-managed and plan-managed participants, at NDIS price limits.
- Support at Home
- Usually referred by the client's care partner (case manager). Billed to the provider at the agreed rate, with the purchase order or service request number.
- DVA
- Gold Card, or White Card for accepted conditions, with a referral from their GP, surgeon or hospital team. Nothing for the veteran to pay.
- GP care plan (Medicare)
- GP chronic condition management plan referrals. They don't need to name us, and they last 18 months.
- Private
- No referral needed. Health fund rebates are claimed on the spot.
- WorkCover or CTP
- Once the claim is approved. Include the claim number.
